WHO WE ARE: We are Maynooth Chiropractic with offices located at Unit F1, Maynooth Business Campus, Maynooth, Co. Kildare, ph 016285962, email: chiromaynooth (at) gmail.com. For the purpose of processing data Dr. Erina Olsen is the “Controller”. 

DATA PROTECTION OFFICER: Your personal data is taken very seriously. The nature of this practice does not necessitate a Data Protection Officer. Should you have any queries regarding your data please contact Dr. Olsen as she is the controller.

THE PERSONAL DATA WE PROCESS AND WHAT WE DO WITH IT: The following categories of personal data are recorded and used: name, address, telephone numbers, date of birth, health information, including medical history, diagnosis and treatment data. Our initial lawful basis of processing this data is one of consent; in addition you will only be examined and treated with your explicit consent. This data will be used to offer effective continuous care through chiropractic services. Continued use of your personal data will fall under “Legitimate Interest” lawful basis of processing as to ensure appropriate continued care. You may, at any time, request that we not contact you in the future. 

RETAINING YOUR PERSONAL DATA: 

Whilst you are undergoing care from this office, your data will continue to be stored and used. Once care ceases your data will be retained for a minimum of 8 years as per the legal requirements. This falls under the “Legitimate Interest” lawful basis of processing. 

YOUR RIGHTS:

As your data is being processed you have certain rights. These are: a right of access, a right of rectification, a right of erasure and a right to restrict processing. You may request a copy of your data at any time. Please make such a request in writing to the controller whose details are listed above. Please provide the following information: your name, address, telephone number and details of the information you require. Your identity will need to be verified so we may ask for a copy of your passport, driving license and/or a recent utility bill. If you believe any of the personal data held on you is inaccurate or incomplete, please contact the clinic directly and any necessary corrections to your data will be made promptly. If you believe we should erase your data, please contact the controller, whose details are shown above. If you wish us to stop storing or using your data, please contact the controller, whose details are shown above. 

SHARING DATA:

We will not share your personal data with marketing companies. 

DATA BREACHES: 

Should your personal data that we control be lost, stolen or otherwise breached, where this constitutes a high risk to your rights and freedoms, we will contact you without delay. We will give you the contact details of the individual who is dealing with the breach; explain to you the nature of the breach and the steps we are taking to deal with it.
